The Nicholas E. Colleluori Financial Aid Fund supports patients who have been diagnosed with blood cancer such as Lymphoma, Leukemia, Multiple Myeloma, Myelodysplastic Syndromes and Myeloproliferative Neoplasms and are undergoing active treatment.  Funding to be used to offset living expenses, travel-related expenses and/or medical expenses.  

Each patient who qualifies must apply below, meet household income guidelines and provide diagnosis documentation in order to finalize the application. Income must be equal to or less than 200% of the federal poverty limits for the current year as indicated in the chart below.  

Once approved, the financial fund provides one-time assistance up to $1,000 (subject to availability of funds) to eligible patients. Applications are reviewed once a month and distributed based on available funds. If selected, notification will be sent out at the beginning of the following month.
